At Goodman Middle School, we believe that sports and activities are an extension of the classroom that help develop well-rounded students. Our programs instill school pride, self-discipline, and vital life lessons while supporting emotional and physical growth. Research confirms that students involved in activities maintain higher grades, better attendance, and fewer discipline problems.

Registration is NOW open in FinalForms for all GMS Sports.

  • Sports Physical fee varies. They are good for 2 years and are required for participation.

 

How to Register for Athletics in FinalForms

Register your student for GMS Athletics by going to Final Forms in order to:

1.) Create your parent account
2.) Add your student(s) to your account and select their sport
3.) Sign and submit your required forms to participate in that sport

4.) Have your student sign into Final Forms  to sign & submit all their forms. 

  • Students will need to use their school email. (student#@edtools.psd401.net)
  • Students can use any computer to log into Final Forms using their school email. (They do not need to only use their school Macbook.)

  • Click the “Get Help” button on the website for FinalForms technical support.
  • Contact our office secretary, Darla Rockstrom, at rockstromd@psd401.net for Goodman support.

This will complete the registration process and your student will be ready to participate in athletics at Goodman Middle School. Before the start of the season, we will verify that all required paperwork has been submitted.

Athlete Physical Health Forms
All student-athletes are required to have current physical health forms on file to participate in school sports. Please print the forms you need and bring them to your physical appointment. Access and complete the necessary forms through the MyWIAA website

ATHLETICS & STUDENT ACTIVITIES

Sport Registration Requirement

  • $20.00 Associated Student Body (ASB) Membership – This annual fee covers all sports participation during a school year.  Our ASB supports all student extracurriculars, including our sports teams! Purchase your student's membership via our PSD bookkeeper's website at this LINK.
  • Sports Physical Form -  They are current for 2 years from the date of the physical. Fee varies with your primary healthcare provider, or you can complete a physical at no cost through Community Health Care Key Medical Clinic. Please call them 253-884-9221 to confirm days and times available for sports physicals.
  • Athlete Physical Health Forms
    All student-athletes are required to have current physical health forms on file to participate in school sports. Please print the forms you need and bring them to your physical appointment. Access and complete the necessary forms through the MyWIAA website
  • Parent & Student Signature Forms -  Completed for each sport season to indicate parent permission and student agreement to District athletics policies. 
  • Academic Eligibility - Student-athletes must maintain a GPA of at least 2.0 (which is a C average) and have no more than one course with an F.

Activities and Clubs

Not all clubs are held each year as each club’s status is dependent on student interest and advisor availability. Information regarding clubs including advisor contacts is listed below when they become available. Clubs typically begin at Goodman in October of each year.  More info to come.
